A Made-in-Canada Surgical Innovation
The multivessel minimally invasive coronary artery bypass grafting technique (known as MICS CABG) was pioneered right here in Canada at the University of Ottawa Heart Institute (UOHI). Led by Dr. Marc Ruel, principal investigator of the study, Endowed Chair and Director of Minimally Invasive Cardiac Research at UOHI, and Professor at the University of Ottawa, the technique avoids splitting the sternum entirely.
Instead, surgeons navigate through a small incision made between the patient’s ribs. By preserving the structural integrity of the chest wall, the procedure dramatically reduces surgical trauma while treating complex, multivessel coronary artery disease.
The Landmark MIST Trial
While minimally invasive bypass surgery has gained traction globally, high-grade evidence comparing it directly against traditional open-chest surgery was previously limited to observational or single-centre studies.
To bridge this gap, Ottawa researchers conceived, designed, and led the MIST Trial (Minimally Invasive versus STernotomy). Published in The Lancet, MIST represents the world’s first randomized controlled clinical trial evaluating MICS CABG against traditional sternotomy.

Key Details of the Trial
- Global Collaboration: The trial enrolled 170 patients across seven medical centres in six countries including Canada, the United States, Germany, Japan, China, and India.
- Complex Multi-Graft Care: Patients who had blockages in multiple heart blood vessels got an average of three bypass surgeries during their operation.
- Randomized Evaluation: Patients were randomly assigned to receive either the Ottawa-developed minimally invasive approach or conventional open-heart sternotomy.
Promising Results for Patients
The findings from the MIST Trial provide strong evidence of the advantages offered by the minimally invasive approach:
- Faster back-on-your-feet recovery: Just one month after surgery, patients who had the smaller incision were moving around and doing physical activities much better than those who had their breastbone cut.
- Easier on the body: Patients who had the minimally invasive procedure needed fewer blood transfusions and were able to get off breathing machines sooner.
- Just as safe and effective: After one year, both groups of patients had the exact same excellent long-term results—with zero deaths, zero strokes, and equal relief from chest pain.
