“Pregnancy involves many changes to the cardiovascular system and is now widely understood as a cardiovascular stress test,” explains Dr. Natalie Dayan, a scientist at The Research Institute of the McGill University Health Centre (MUHC) and founder of its postpartum maternal cardiovascular health clinic.
“For some women, complications during pregnancy like preeclampsia or severe obstetric hemorrhage unmask an underlying vulnerability for heart disease.”
In fact, when Dr. Dayan evaluated patients in her clinic shortly after delivery, the findings were eye-opening: “One third of women seen in this clinic have an underlying metabolic abnormality identified during their first visit at about 3 to 6 months after delivery. However, it has become evident that we need a more robust and systematic way to quantify risk.”
What Happens to the Body During a Difficult Pregnancy or Multiple Miscarriages?
Why do complicated pregnancies leave a lasting mark on a woman’s cardiovascular system? Cardiologists and obstetricians view pregnancy as a natural “stress test” for the heart. If an underlying vulnerability exists, the physical demands of pregnancy can bring it to the surface, and in some cases, cause lasting cellular and vascular changes:
- Endothelial Strain and Vascular Damage: Conditions like preeclampsia and gestational hypertension damage the endothelium (the inner lining of blood vessels). This inflammation can cause blood vessels to remain stiffer and less flexible long after delivery, setting the stage for early high blood pressure and plaque buildup.
- Metabolic Shifts: Gestational diabetes signals that the body’s insulin response is under strain. Even after blood sugar levels return to normal post-birth, the temporary metabolic stress increases the long-term risk of insulin resistance and type 2 diabetes, both major risk factors for heart disease.
- Chronic Inflammation and Immunity: Multiple miscarriages and pregnancy loss may be tied to systemic inflammation, clotting disorders, or autoimmune responses. Chronic inflammation damages blood vessels over time, making arterial walls more prone to hardening later in life.
What About Women Who Have Never Been Pregnant?
A common question arises: If pregnancy acts as a cardiovascular stress test, does never being pregnant mean a woman has lower heart disease risk?
Not necessarily. While women who have never been pregnant do not undergo the physical stress test of pregnancy, they are not immune to cardiovascular risk:

Hormonal conditions like Polyendocrine Metabolic Ovarian Syndrome (formerly named PCOS, now PMOS), early menstruation (before age 11), or early menopause (before age 40) are all highlighted by the U.S. Centers for Disease Control and Prevention (CDC) and increase heart disease risk regardless of pregnancy history.
- The Research Gap: Historically, cardiovascular studies have focused primarily on older male populations, leaving a gap in understanding how non-pregnancy reproductive factors accumulate over a woman’s lifespan. Current research continues to investigate how lifetime estrogen exposure and hormonal cycles impact long-term heart health in women who have never been pregnant.
The McGill Research: A New Score for Women’s Hearts
To address these critical gaps, a Canada-wide research team led by Dr. Dayan and doctoral candidate Melia Alcantara analyzed national health data from nearly two million births between 2008 and 2021.
Published in JACC: Advances, their study created a pioneering risk score that combines standard health factors with pregnancy-related complications.
“Our goal is to develop a tool that reflects the full clinical picture after pregnancy, not just traditional heart disease risk factors, but also complications that occur during pregnancy itself,” says first author Melia Alcantara, M.Sc.. “By combining these elements, we were able to create a risk score that can help stratify patients into low, moderate and high-risk categories, representing a promising first step toward a clinical risk calculator.”
The findings were clear: while only 1.5% of women fell into the highest-risk group, those women were nearly eight times more likely to be hospitalized for cardiovascular disease within one year of giving birth compared to baseline rates.
Red-Flag Pregnancy Factors You Should Never Ignore
If you have experienced any of the following during your reproductive years, your heart requires closer attention:
- Preeclampsia or Gestational Hypertension: High blood pressure during pregnancy doubles your long-term risk of heart disease and stroke.
- Gestational Diabetes: Blood sugar spikes during pregnancy increase the likelihood of developing type 2 diabetes and long-term arterial stiffness.
- Preterm Birth: Delivering before 37 weeks is linked to an increased risk of future cardiovascular events.
- Severe Obstetric Hemorrhage: Heavy bleeding during or after delivery can indicate underlying vascular vulnerabilities.
- Multiple Miscarriages or Pregnancy Loss: Repeated pregnancy loss is an early marker of systemic vascular and inflammatory factors that affect heart health over time.
How to Protect Your Heart at Any Age
The good news is that identified risk is not a destiny. According to guidelines from the CDC and heart health experts, women can take powerful, proactive steps to lower their risk:
- Monitor Your Key Numbers: Keep track of your blood pressure, cholesterol, and blood sugar levels starting early in adulthood. Blood pressure readings of 130/80 mm Hg or higher warrant conversation and management with your doctor.
- Share Your Complete History: Ensure your primary care doctor, cardiologist, and gynecologist where applicable know your full reproductive history, including pregnancy complications, miscarriages, PCOS, or early menopause.
- Stay Physically Active: Aim for at least 150 minutes of exercise (like brisk walking, swimming, strength training cycling – or a mixture of all) each week. Even brief sessions benefit blood vessel elasticity.
- Prioritize Nutrition & Stress Management: Focus on a heart-healthy diet rich in whole grains, vegetables, lean proteins, and healthy fats while limiting sodium, processed foods, and alcohol.
- Avoid Smoking: Tobacco use severely accelerates vascular damage, particularly in women with a history of high blood pressure or preeclampsia.
“The postpartum period is a critical opportunity to educate and intervene,” emphasizes Dr. Dayan. “But we need better tools to guide who requires more intensive follow-up and risk factor management—and who can be reassured about their heart health. This risk score is an important first step to realizing this goal.”
Dr. Dayan notes that the ultimate goal is practical: “We hope this work will enable clinicians and their patients to develop tailored, evidence-based strategies to improve heart health based on individual risk.”
Women’s heart health has traditionally been woefully misunderstood, with medical care relying on tests and models built primarily around men. But the tide is turning. Breakthroughs like McGill University’s new risk tool prove that medical science is finally recognizing a woman’s unique biology, starting with her pregnancy history.
